I hereby request the Village of New London, Ohio to supply water to the Address of account shown in this application. As a condition of this request, and in consideration of any water service, which I may receive, in signing this application below, I hereby agree to the following: 1. I agree to pay for same in accordance with the schedule of water rates, tap-in fees, and charges as they are established by the Village of New London, Ohio and which may, from time to time, be amended on file in the utility office. 2. I agree to comply with the rules and regulations governing such services, including but not limited to all of the Ordinances of the Village of New London concerning water usage, as said Ordinances of an approved backflow device is required. 3. I agree to be fully responsible for the service line from the water meter/cut-off to the home/building, including the installation of an approved backflow device if required. 4. I agree that the Village of New London shall have final authority in any question of location of any service line connection to its water main line; shall determine the allocation of water to Applicants in the event of a water shortage; and may shut-off water to an Applicant who allows a connection or extension or extension to be made of the Applicant’s service line for the purpose of supplying water to another user. 5. I agree that, in the event the total water supply shall be insufficient to meet all the needs of the Applicants, or in the event there is a shortage of water, the Village of New London may prorate the water available among the various users on such schedule of hours covering use of water for garden purposes; provided that, if at any time the total water supply shall be insufficient to meet all of the needs of all the users, the Village of New London may first satisfy all of the needs of all users for domestic and livestock purposes before supplying any water for garden purposes. 6. I agree that the Village of New London, its successors and assigns shall have a license and permission to enter onto the premises at the address of account to perform meter/usage readings and to inspect any pipelines connected into the Village’s pipelines. 7. I agree that my failure to timely pay water charge duly impose shall result in the automatic imposition of the following: A. Assessment of any late charge or fee authorized by the Village Ordinances. B. Water being shut off from the Applicant’s property. C. In the event it becomes necessary for the Village of New London to shut off the water from an Applicant’s property, a $50.00 fee set by the Village of New London in its rate schedule will be charged for a reconnection of the service during normal business hours, outside normal business hours and weekends the reconnection fee is $75. 8. If this application is for a location that has not previously been served by the Village of New London, I acknowledge and agree to the following conditions: A. The Village of New London will install at the Applicant’s expense a 5/8 X ¾ inch water meter, a cutoff valve and a ¾ inch service line, which shall begin at the water main line and extend to the property line. The Village of New London shall have exclusive right to use such cutoff valve and water meter. B. The Applicant shall install and maintain at the Applicant’s expense a service line, which shall begin at the meter/cutoff and extend to the dwelling or place of use. The service line shall connect to the Village of New London’s water meter/cutoff. C. The Applicant agrees that no other present and future source of water will be connected to any water lines served by the Village of New London’s water lines and will disconnect from the present water supply prior to connecting to and switching to the Village of New London’s system and shall eliminate any present and future cross-connections to the Applicant’s system. D. The Applicant shall connect the service lines to the Village of New London’s water meter and shall commence to use water from the system on the date the charges to the Applicant shall commence on the date service is made available, regardless of whether the Applicant connects to the system. E. The Applicant agrees to grant to the Village of New London, its successors and assigns, if necessary, a perpetual easement in, over, under and upon the above described land, with the right to erect, construct, install, and lay, and thereafter use, operate, inspect, repair maintain, replace, and remove water pipelines and appurtenant facilities, together with the right to utilize adjoining lands belonging to the applicant for the purpose of ingress to and egress from the above described lands. 9. I acknowledge that my submitting this application for water service is subject to the Applicant’s being in good standing with respect to any prior outstanding balance due to the Village and the Village’s determination that it has the capacity and ability to service Applicant’s premises. 10. I acknowledge that my signing this application, for the Village of New London to supply utility services to the stated address on the application, I hereby agree to accept the Village of New London Utility Rules and Regulation. I also acknowledge that I have received a copy of the Utility Rules and Regulations.
